What paperwork do I need to provide with the form?

If you are a property owner and the work was carried for you, you need to submit a:

  • Copy of letter or email sent to the contractor notifying them of all items
  • Contract, quotation or evidence to identify the contractor as the person responsible for the work (if you don't have a written contract or quote, you can provide proof of payment for the work e.g. receipts or bank statements identifying specific payments made to the contractor).

If you are a property owner and the work was done before you purchased the property, you need to submit a:

  • Copy of letter or email sent to the contractor notifying them of all items
  • Purchase contract (Contract of Sale) including any special conditions
  • Pre-purchase inspection report/s (if available).

If you are a body corporate representative (for common property only), submit a:

  • Copy of letter or email sent to the contractor notifying them of all items
  • Certificate of Classification
  • Complete Survey Plan
  • Community Title Scheme document

If you are an agent acting on behalf of a property owner, submit a:

  • Copy of letter or email sent to the contractor notifying them of all items
  • Copy of letter or email signed by the property owner authorising agent to act on their behalf
  • Contract, quotation or evidence to identify the contractor as the person responsible for the work.
  • If you don't have a written contract or quote, you'll need to provide proof of payment for the work e.g. receipts or bank statements, identifying specific payments made to the contractor

If you are an owner of the property directly next door, submit a:

  • Copy of letter or email sent to the contractor notifying them of all items.
