Lodge this form if you wish to complain about a non-conforming building product, including a suspected non-conforming building product, or related matter.

When do I lodge the complaint?

As soon as practicable.

Notification of a non-conforming or suspected non-conforming building product (by person in the chain of responsibility)

This form is for a person in the chain of responsibility for a building product who becomes aware, or reasonably suspects, that the building product is a non-conforming building product for an intended use, to give notice of the matter in accordance with section 74AL of the Queensland Building and Construction Commission Act 1991.

When do I lodge the complaint?

Notice of the matter must be given to QBCC as soon as practicable but within 2 days after becoming aware or reasonably suspecting.

Notification of notifiable incident caused by a non-conforming building product (by person in the chain of responsibility)

A notifiable incident means—

the death or serious injury or illness of a person; or

an incident that exposes a person to a risk of serious injury or illness, such as the collapse or partial collapse of a building.

This form is for a person in the chain of responsibility for a non-conforming building product who becomes aware, or reasonably suspects, that a notifiable incident was or may have been caused by the use of the product to give notice of the matter in accordance with section 74AM of the Queensland Building and Construction Commission Act 1991.

When do I lodge the complaint?

Notice of the matter must be given to QBCC as soon as practicable but within 2 days after becoming aware or reasonably suspecting.

Notification of particular safety matters (by licensee)

A notifiable incident means—

  •  the death or serious injury or illness of a person; or
  • an incident that exposes a person to a risk of serious injury or illness, such as the collapse or partial collapse of a building.

This form is for completion by a licensee who becomes aware of either of the following (each a safety matter)—

  • a notifiable incident has occurred on a building site under the licensee’s control or on which the licensee is carrying out building work;
  • a person carrying out building or other work on a building site under the licensee’s control, or on which the licensee is carrying out building work, has contravened or is contravening—
    • a notice or injunction under the Work Health and Safety Act 2011, part 10; or
    • a notice or injunction under the Electrical Safety Act 2002, part 11A.

    Please note that these QBCC NCBP forms do not fulfil further requirements to report incidents of a Workplace Health Safety issue on a building site with WorkCover Queensland or any issues Queensland Health need to be aware of, if you require these services please use the links below.

    A workplace incident that falls into one of the below categories is required to be notified to the Office of Fair and Safe Work Queensland under the Work Health and Safety Act 2011, the Safety in Recreational Water Activities Act 2011, or the Electrical Safety Act 2002.
